Required Documents / आवश्यक दस्तावेज़
- Mandatory submission before burial
दफनाने से पहले निम्नलिखित दस्तावेज़ों को जमा करना अनिवार्य है:
- Declaration of Cause of Death by a registered physician (e.g., Medical Certificate of Cause of Death on Form 4/Form 4A- for deaths in the hospital, OR a doctor’s letter-for deaths at home)
मृत्यु के कारण की घोषणा: एक पंजीकृत चिकित्सक द्वारा जारी प्रमाण पत्र (जैसे कि अस्पताल में मृत्यु के लिए फॉर्म 4/4A पर मृत्यु के कारण का मेडिकल सर्टिफिकेट, या घर पर मृत्यु के मामले में डॉक्टर का पत्र)।
- Church/Congregation/Pastorate/Parish Membership Letter from the Pastor/Presbyter/Parish Priest.
सदस्यता पत्र: संबंधित पादरी/प्रेस्बिटर/पैरिश पुरोहित द्वारा जारी चर्च/कांग्रिगेशन/पास्टोरेट/पैरिश सदस्यता का पत्र।
- Copy of the deceased’s Aadhaar Card or Passport.
मृतक का पहचान पत्र: मृतक के आधार कार्ड या पासपोर्ट की प्रति।
- Copy of a surviving family member’s Aadhaar Card or Passport.
परिवार के सदस्य का पहचान पत्र: जीवित परिवार के सदस्य के आधार कार्ड या पासपोर्ट की प्रति।
Burial Fees / बरिअल फीस (दफ़नाई शुल्क)
- Fees support cemetery and grave upkeep. To be contributed as donations follows:
यह शुल्क कब्रिस्तान और कब्रों के रखरखाव के लिए लिया जाता है। इसे निम्नलिखित रूप में जमा करना होगा:
- Church of the Epiphany members: ₹10,000 donation for a kutcha grave (pay at Church Office before proceeding with funeral; receipt issued).
चर्च ऑफ द एपिफनी के सदस्यों के लिए: कच्ची कब्र के लिए ₹10,000 का दान (दफ़नाई से पहले चर्च कार्यालय में जमा करें; रसीद जारी की जाएगी)।
- Members of other Protestant Churches: ₹15,000 donation for a kutcha grave (pay at Church Office before proceeding with funeral; receipt issued).
अन्य प्रोटेस्टेंट चर्चों के सदस्यों के लिए: कच्ची कब्र के लिए ₹15,000 का दान (दफ़नाई से पहले चर्च कार्यालय में जमा करें; रसीद जारी की जाएगी)।
- All burials: Additional ₹2,000 cash to the Cemetery Caretaker on the day of burial.
सभी दफन प्रक्रियाओं के लिए: दफनाने के दिन कब्रिस्तान के केयरटेकर (रखवाले) को ₹2,000 नकद अतिरिक्त देने होंगे।
N.B.: This SOP emphasises pre-verification to meet regulatory standards and are in compliance with authorities’ directives on verifying the deceased’s identity, cause of death and Christian affiliation, the below mentioned documentation shall now be strictly adhered to.
विशेष नोट (N.B.): यह SOP नियामक मानकों को पूरा करने के लिए पूर्व-सत्यापन (pre-verification) पर जोर देती है। मृतक की पहचान, मृत्यु के कारण और ईसाई संबद्धता को सत्यापित करने के संबंध में अधिकारियों के निर्देशों के अनुपालन में, अब ऊपर उल्लिखित दस्तावेज़ों का कड़ाई से पालन किया जाएगा।
